CAPMT OC Chapter Community Recitals Application Form

  1. Each student limited to one piece, unless the piece is less than one minute in length - in which case the student may play two pieces. Teachers limited to 8 students and/or student  ensemble groups per recital.
  2. Entry fee is $5.00 ($10.00 if non-CAPMT OC member)  per student (non-refundable).  One check per studio (from teacher, not parent), payable to “CAPMT OC Chapter”
  3. Type or clearly print names and full titles, ensuring correct spelling and complete information. All solo pieces must be memorized.
  4. Be sure each student receives a copy of the Recital Guidelines and Policies. Students are expected to have proper recital etiquette and attire.
  5. Chair must receive completed form with payment ten (10) days in advance of scheduled recital date (postmark okay).  Send the application (may be submitted online) and one check  to the address below.
